Community Therapy Support Worker

Job summary

This is an exciting opportunity for a rehab support worker to work within our established In Patient (Pathway 2) team within the Amber Valley and Erewash area.

The Pathway 2 therapy team provides timely and appropriate care to patients within our local pathway 2 beds.

Our Amber Valley and Erewash Integrated Community Services are a multi-disciplinary team, providing person-centred holistic care to patients with a wide variety of health conditions.

The Community Therapy team comprises of Physiotherapists and Occupational Therapists with band 4 and band 3 support workers. We are part of a large Integrated Community Team which include Community Nursing, Care Co-ordinators and the Community Response Team.

You will be required to work weekends as part of a shared rota. This will give you the opportunity to see urgent patients as part of a wider team. You will receive enhanced weekend pay and days off in the week as a result (approx 1 in 6 weekends).

Main duties of the job

You will be required to follow treatment plans within the patient record and under the direction of the qualified Therapists, deliver Therapy intervention to patients.

The post holder will work alongside Therapists and Social Care partners to facilitate reablement and Discharges in a timely manner to enable flow through our in patient services.

You will be required to do an array of mandatory training related to your role and we will identify any gaps in your knowledge and skills and support you to close these to gain your competencies as a Community Therapy Support Worker.
