Job summary
We currently have an exciting opportunity for a part time Community Response Team Clinician to join us!
The CRT Clinician is a Band 6 clinical role responsible for coordinating patient flow across Chesterfield and North East Derbyshire. The clinician works autonomously to assess patients, triage referrals, and ensure timely and appropriate care pathways. This includes managing referrals from healthcare professionals and navigating patients to the most suitable care package available.
Main duties of the job
1. Clinical Assessment and Navigation
- Conduct comprehensive assessments of patients referred to the CRT.
- Determine discharge plans or transfers to other care settings.
- Clinically navigate referrals to appropriate services, prioritising based on urgency and need.
2. Multidisciplinary Collaboration
- Work closely with health and social care teams to develop and implement care packages.
- Participate in daily communications and weekly caseload reviews to ensure continuity and quality of care.
3. Documentation and Communication
- Maintain accurate records in line with professional guidelines.
- Use systems like SystmOne and the Community Access Point (CAP) for documentation
4. Leadership and Supervision
- Lead urgent visits and delegate tasks to support workers.
- Provide supervision, appraisals, Sickness monitoring and mentorship to junior staff as per trust guidelines

Additional Duties
- Attend daily flow calls with Adult Care colleagues.
- Offer CRT capacity for joint or shared care packages with START teams
- Ensure all patients have active care plans and that visits are scheduled and completed, including weekend coverage
