As a Care Coordinator in the dynamic Joined Up Care Derbyshire 'Care Transfer Hub', you will be at the heart of a team dedicated to processing intermediate care referrals.
Your role will be pivotal, acting as the essential link between service users, carers, primary care, secondary care, community care, social care, mental health services, out of hours services, and voluntary organisations.

Main duties of the job
Monitoring a caseload within the Care Transfer Hub to track a person's journey and outcomes - escalating delays or concerns to the multi-disciplinary team.
Chasing additional information and updates to referrals, ensuring this is documented in the shared database.
Liaising with organisations across the Joined Up Care Derbyshire system to identify service availability and provide updates.

The Care Transfer Hub is a newly developed service to facilitate timely access to intermediate care. This role will support the hub team to navigate, coordinate and plan care for the people of Derbyshire. This is an exciting time to join the team and be involved in the development of the Care Transfer Hub and its processes.
