We currently have an exciting opportunity for a Registered Nurse to join our established community nursing teams based at Ilkeston Community Hospital on a fixed term basis covering a secondment. This role will provide care for housebound patients in the Erewash locality, including Ilkeston and the surrounding areas.

Main duties of the job

Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurse in the management of their clinical caseload.

Implement / refer on to the relevant services as appropriate.

Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
