Job summary

There is flexibility available for the successful applicant to be employed by either Derbyshire County Council, or Derbyshire Community Health Services NHS Trust - and the post is being advertised to INTERNAL applicants only and whom are currently working for any organisations within Joined Up Care Derbyshire (the integrated care system).

Derbyshire County Council and Derbyshire Community Health Services NHS Trust have recently agreed to explore integration of a range of services that support people to remain at home when their needs change, but also to return home following a hospital admission by receiving rehabilitation and reablement support. Both organisations have been committed to working together for a number of years and we already deliver multi-professional assessments, are flexible in how we use our workforce and we share good practice on training and development. We are now pushing forward to achieve a greater level of "joined up" care for individuals and are looking for a highly ambitious and experienced Programme Manager who can help us take forward a complex and wide ranging programme of work to move us to the next stage.

This post is a unique position in that it will work to lead and manage a programme of work across both the Council and the Trust, being jointly responsible to both organisations' Senior Management teams.

Main duties of the job

The post holder will be expected to play a key role in working to the joint Programme Directors to ensure that both organisations are prepared and ready for a newintegrated partnership. This will mean devising a programme plan and coordinating a variety of workstreams associated with this work, including scoping out the potential future operational delivery opportunities with the relevant Operational Directors and their teams, during the programme development phase. The post holder will be expected to have oversight of the workstreams to ensure all component parts are driven forward by relevant project leads in the required timescales and to the required standard. They will be expected to draft highlight reports and keep a clear log of all risks and issues.

About us

This post is offered on a fixed term basis, with a review to be held at the end of March 2026 to determine the overall progress of the programme and whether there is a need for a further extension of the role. The post involves hybrid working but candidates must be available to work flexibly within the geography of Derby and Derbyshire.
