Job summary

An exciting new opportunity has arisen for a highly motivated and enthusiastic PCN Care Co-ordinator, to work across South PCN as part of our Integrated Community Team in Derby City.

Key aspects of the role will involve working closely with GP's, Enhanced Clinical Practitioners and wider MDT, Co-ordinating, identifying and supporting patient in their own homes who require additional support and proactive care planning in line with the Ageing Well agenda.

The successful applicant will have excellent communication skills, be able to work independently and be confident using IT systems.

Main duties of the job

The PCN Care Coordinator will be part of the Home Visit Service who will be responsible for coordinating and allocating visit and also actioning any required referrals.

The PCN Care Coordinator will also work along side the Enhanced Care Home support team who are responsible for managing planned long term care for patients within Residential and Nursing Care Homes.

The PCN Care Coordinator are pivotal and are the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

To maintain IT based information systems and take responsibility for the production of relevant performance data.

The Care Co-ordinator will contribute to tackling inequalities in health and social care particularly regarding individuals with long-term conditions. An ethos of promotion of independence and partnership-working is integral to this post

  • ECDL or equivalent
  • NVQ Level 3 Business Administration, or equivalent
  • Diploma/ HNC level (or relevant, equivalent experience)

Experience and Knowledge

  • Able to demonstrate a clear understanding of working with confidential information
  • Recent and relevant experience of working with healthcare professionals/previous experience in the NHS or social care
  • Working in a multi-disciplinary setting where influence and negotiation is required
  • Experience in use of databases
