Come and join the Amber Valley Early Supported Stroke Discharge Team (ESSD) for a fixed term period ending 31st December 2025.

Our team focus is on community rehabilitation of patients following a Stroke, working as part of the MDT.

It is essential that you have a keen interest and some experience in this field.You will have excellent communication and problem solving skills and the ability to plan and organise competent, evidence based clinical care.

We will provide you with a supportive environment to develop your skills and experience. Our evolving service has a strong emphasis on high quality, evidence based care, team work and staff well-being.

We welcome applications from those already at Band 6 and also current Band 5 OTs with relevant experience, knowledge and skills who are looking to develop into a Band 6 role.

You will be required to travel to reach patients in the community and you must be able to access transport.

Main duties of the job

We have an exciting offer available for an Occupational Therapist to gain or increase their skills in stroke rehabilitation working with our Early Supported Stroke Discharge team (ESSD) for a fixed term period ending 31st December 2025.

During your time with us we will provide mentor and development support, we have a strong network of highly skilled Occupational Therapists across the service and within the team itself. Our aim would be to give you a positive and rewarding year with clinical experience, knowledge and skill development and support.

There are 3 ESSD teams who work closely alongside each other to provide care to people in South Derbyshire, Derby City and Amber Valley. This position would be predominantly based with the Amber Valley team, but where there are capacity needs, you may be required to work flexibly to support the other teams.

The ESSD team is a friendly and supportive team who look forward to supporting a new colleague to developing skills and knowledge in the clinical area they are passionate about.

Essential

  • Bsc / diploma in Occupational Therapy
  • HCPC registration
  • Ability to travel in the Community
  • Documented evidence of Continuing Professional Development
  • Postgraduate experience and /or training in Stroke rehabilitation

Desirable

  • Membership of Professional Body and Special Interest Groups particularly Neurological Practice

Experience

Essential

  • Minimum of 4 years post graduate experience
  • Evidence of experience treating strokes
  • Experience of working in physical rehabilitation (all ages).
  • Experience of working with carers
  • Experience of multi-disciplinary team working

Desirable

  • Minimum of working for 2 years in community setting
  • Experience of work within ESSD setting

  • Experience acquired skills in stroke rehabilitation (Including e.g. normal movement, Bobath)
  • Use the OT process and associated models, frames of reference and approaches
  • Ability to work as part of a multi-disciplinary team.
  • Ability to undertake cognitive/perceptual assessment/rehabilitation
  • Ability to maintain documentation that is relevant, concise, accurate and legible.
  • Excellent communication skills and good time management skills
