Job summary

We have a new and exciting position for a Service Manager to oversee and develop Derbyshire Community Health Services NHS Foundation Trust's (DCHS) General Practice Service (GPS) portfolio.

The service currently comprises of four practices, operating from five sites, which sit within three Primary Care Networks. With approximately 80 staff and a budget of £3m, the service serves a registered population of around 20,000 patients. A good understanding of general practice, how they are commissioned and operate will be important.

The Service Manager will lead the drive on the service's mission, for "a compassionate, competent, and sustainable model of General Practice that makes a positive contribution to the lives of our patients and workforce".

We are looking for a skilled and dynamic leader, with expertise in managing teams across multiple sites, service improvement and transformation. The service's General Manager and Clinical Director are seeking for the set of practices to work in a more integrated, standardised and sustainable manner, and this transformation will be a key priority for the Service Manager during the first 12-18 months.

The role will require travel between sites and to attend meetings, including to areas with poor public transport links, and so the ability to drive will be beneficial.

Main duties of the job

You will report, and be accountable, to the General Manager.

You will be responsible for the professional and operational leadership and improvement of the service.

You will be responsible for the effectiveness of the service, ensuring the delivery of high quality, patient centred, integrated, cost effective care across the locality/service, having leadership responsibility for the teams.

To be responsible for the provision, co-ordination and development of services, interpreting relevant SOPs, policies and pathways and working with the General Manager to promote and foster partnership working across the Derbyshire system with commissioners, Primary Care Networks and other key stakeholders.

You will provide operational and professional leadership (within a multi-disciplinary team) to the relevant direct reports, clinical teams and team leaders and be responsible for the development and implementation of quality and performance monitoring arrangements.

To effectively apply and lead a matrix approach across the organisation when developing quality outcomes, the core capabilities, knowledge and attitudes when considering care delivery.

Overseeing complex operational issues including workforce planning, change management and workforce engagement.

About us

DCHS employs approximately 4,400 people, making us one of the largest providers of specialist community health services in the country. We serve a patient population of more than 1.1 million people across Derbyshire and Derby and have an annual budget of £200mn.

We became a standalone NHS organisation in 2011 and a Foundation Trust with greater autonomy from 2014.
