The Intensive Support Team (IST) has a full time vacancies for Band 5 Staff Practitioner, based in Chesterfield and covering the North Derbyshire geographical area.

We are looking for enthusiastic and motivated individuals who are passionate about improving the lives of people who have learning disabilities.

The IST is part of the multi-disciplinary LD Community Services.

We also work closely with care providers, social care and inpatient units.

We focus on the individual and aim to provide responsive, intensive and personalised care, to avoid inappropriate and high cost hospital admissions.

We work closely with carers, families and our MH and primary care colleagues to maintain individuals in their own home and engaged in their own community.

Main duties of the job

You will be an important member of our multi-disciplinary team.

You will have knowledge and experience in a variety of areas, including behaviour, mental health issues and common conditions such as Autism.

You will be expected to work with service users, families, carers and other agencies, as well as supporting students during placements.

If you can demonstrate excellent communication skills, flexibility and are able to plan and deliver high quality, person-centred, evidence-based clinical care, we would very much like to hear from you.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

  • Recent and relevant post-registration experience
