Job summary

An opportunity has arisen at Ripley Medical Centre for a Registered Nurse to join our friendly Practice. This presents an ideal opportunity for the right candidate to join an established service within a supportive team and forward-thinking practice.

Ripley Medical Centre is part of Derbyshire Community Health Services NHS FT General Practice Service, alongside 3 other established Practices, and we are looking for an experienced, flexible, and highly motivated individual to join our team. Development is encouraged and supported by DCHS and the Practice to further career progression with various training opportunities available.

Ripley Medical Centre is a large practice of over 6,000 patients and use TPP SystmOne Clinical system.

CQC rated Good in all areas in 2016, alongside DCHS rated as an Outstanding NHS Trust in 2019.

Multi-disciplinary clinical team of Salaried GPs, Advanced Clinical and Care Practitioners, Practice Nurse and HCA team, we have a strong educational ethos; Training Practice for GP Trainees and Medical Students with accredited trainers, but with all clinicians taking an active interest and contributing to training.

Main duties of the job

Assist the whole practice team to help deliver effective implementation of management of patients and their needs.

Managing the nursing care of patients with chronic disease, employing recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract purposes is also required.

Contribute to the development of policies / protocols / guidelines within the practice.

Adhere to infection control measures within the practice according to local and national guidelines.

Aid the practice team with implementation of new initiatives in the delivery of chronic disease management services.

Encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working.

Recognise the opportunities for working in partnership with the Primary Health care team.
