The Intensive Support Team (IST) are looking for Band 6 Lead Nurses who will be based at Ash Green LD Centre, Chesterfield, covering the North Derbyshire geographical area.

These posts are open to Registered Nurses who hold qualifications in Learning Disability or Mental Health Nursing.

We are looking for enthusiastic, motivated individuals who are passionate about improving the lives of people who have a learning disability.

Please be aware that the Intensive Support Team cover 7 days a week , 52 weeks a year; between the hours of 8.00am and 8.00pm.

A variety of shift patterns are available, including 8.00am-4.00pm, 12.00noon-8.00pm and long days. Exact details can be negotiated with the right candidate.

Main duties of the job

The IST is part of the multi-disciplinary Derbyshire Adult Neurodevelopmental Services.

We work closely with a variety of stakeholders - Individuals, their families and carers, colleagues in Adult Care, Primary Care, Specialist Care Providers, as well as our inpatient Assessment and Treatment Unit.

We focus is on the individual and aim to provide responsive, intensive and personalised care, to avoid inappropriate and high cost hospital admissions.

We work to maintain individuals in their own home and engaged in their own community, and follow the aims of the NHS 'Building the Right Support' Action Plan.

About us

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire ICB.During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health andsocial care.
