The Intensive Support Team (IST) are looking for Band 6 Lead Nurses who will be based at Ash Green LD Centre, Chesterfield, covering the North Derbyshire geographical area.
These posts are open to Registered Nurses who hold qualifications in Learning Disability or Mental Health Nursing.
We are looking for enthusiastic, motivated individuals who are passionate about improving the lives of people who have a learning disability.
Please be aware that the Intensive Support Team cover 7 days a week , 52 weeks a year; between the hours of 8.00am and 8.00pm.
A variety of shift patterns are available, including 8.00am-4.00pm, 12.00noon-8.00pm and long days. Exact details can be negotiated with the right candidate.
Main duties of the job
The IST is part of the multi-disciplinary Derbyshire Adult Neurodevelopmental Services.
We work closely with a variety of stakeholders - Individuals, their families and carers, colleagues in Adult Care, Primary Care, Specialist Care Providers, as well as our inpatient Assessment and Treatment Unit.
We focus is on the individual and aim to provide responsive, intensive and personalised care, to avoid inappropriate and high cost hospital admissions.
We work to maintain individuals in their own home and engaged in their own community, and follow the aims of the NHS 'Building the Right Support' Action Plan.

You will have specialist knowledge and experience in a variety of areas, including communication, behaviour, mental health issues and other common conditions such as Autism.If you can demonstrate excellent communication skills, flexibility and are able to plan and deliver high quality, person centred, evidence-basedclinical care, we would very much like to hear from you.
