We have a fantastic opportunity for a qualified District Nurse to work in our Amber Valley, Heanor community nursing team.
This is an ideal post for a registered nurse with extensive community experience who holds the Special Practitioner Qualification - District Nurse.
The successful applicant will be working alongside a second District Nurse within the Heanor Team and would have the support of other DNs across the patch Alfreton and Ripley (Amber Valley).
The post is full-time (37.5 hours). Predominantly, you will work Monday-Friday, covering weekends on a rolling rota.
Main duties of the job
The successful applicant will hold the District Nurse (SPQ) qualification with great knowledge of caseload management, a high standard of community nursing knowledge and clinical skills.
You will be provided with training and support tailored to your needs to drive forward high quality care and team work; including a thorough induction programme, regular one to ones, peer supervision, working within an MDT and clinical training.
You will have excellent peer support from experienced District Nurses covering the other ARCH locality (Alfreton, Ripley and Heanor). As well as our buddy teams over in Belper and in Rapid Response.
You will provide effective evidenced based care to a defined population/Primary Care Network. This is achieved through effective delegation, case load management, monitoring of care standards, leadership and the performance management of team members.
To assess, plan, implement and evaluate individualised patient care, utilising their professional judgement at all times.
Provide clinical leadership and line management to the community nursing team, and other members of the multidisciplinary team in the delivery of high quality care.
To contribute to the continuous improvement of community nursing within their Place, linked to the 10 year NHS plan.
