Job summary
We are seeking to recruit an enthusiastic and motivated Healthcare Assistant/Trainee Nursing Associate to join our team based at Village Community Medical Centre or Coleman Health Centre in Derby City.
We have various posts available, with a of a minimum of 22.5 hours per week up to a maximum of 37.5 hours per week (full time).
You will be required to travel across Derby City, as part of a wider integrated team providing holistic care to patients in their own homes. Varying hours may be available in other teams within the Derby City locality.It is expected that you will have experience working in either a health or social care setting.A good standard of English is required, along with good communication skills.
As a clinical employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way.
Main duties of the job
This post will involve working within a community nursing team visiting patients within their own homes.
The post holder will:
- assess, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
- provide the delivery of high-quality care and actively support the District Nurse in the management of the health care environment.
- implement the relevant service.
- work in close collaboration with the Integrated Community Teams.

- To provide holistic care to patients in their own homes or alternative health care setting, under the supervision of the appropriate registered professional.

- To contribute to and support the maintaining of service standards and performance through audit and benchmarking, as per internal audit priorities, CQUIN, CQC Standards for Compliance etc; and to encourage colleagues and team members to do also.
- To participate, as an individual and as team member, in continuous learning and reflective practice in order to maintain own knowledge and skills and share good practice with colleagues to continually improve services.
- To actively contribute to protecting people at risk by assessing and monitoring the care environment and individuals and by following and acting on and within Safeguarding policies and procedures.
