Job summary

Shares responsibility for the delivery of home visits across the PCN in the community with medical, nursing, therapy and other specialist colleagues; in a patient-centred and clinically-effective way; and in line with Derbyshire Community Health Services (DCHS) and the PCN North Hardwick and Bolsover's policies and procedures or national guidelines.

As an expert Advanced Practitioner, the post holder will work in a multi-disciplinary team in providing house bound clinical care and have a clinical caseload with input from medical practitioners.

The post holder will establish, co-ordinate and deliver direct clinical care that is diverse, evidence-based and reflective of need.

In line with the implementation of non-medical-led services, the post holder will be a strong leader in the strategy for redesign of services and workforce planning focusing on leadership, accountability, support, consultation, career development, continuing professional development and innovation.

The post holder will provide advanced theoretical and practice knowledge in the field of advanced nursing practice.

They will lead key training strands to facilitate the development of the wider MDT team, community matrons and primary care teams.

Main duties of the job

  • Demonstrate a high degree of professional nursing skill, competency and multidisciplinary knowledge in the area of care of the elderly. The post holder will be responsible for and manage a complex caseload of inpatients.
  • Assess patients requiring admission from community and develop working relationships with community teams advising as and when appropriate on suitable treatment plans.

About us

To work as part of a small team of Community General Practitioners and Doctors, to support multi-disciplinary teams for North Hardwick and Bolsover PCN.

This is direct work with patients and their carers either in person or remotely, however the main remit is to support the clinical practice and development of the Team Up service working with other clinicians.

To assist with and perform the provision of holistic, high quality, evidenced based health care to a defined local population, working within an appropriate supervisory and monitoring framework, to an agreed, multi-disciplinary treatment plan, by providing ongoing treatment, teaching and instruction to their own referred caseload of patients.
