Job summary

We have an exciting opportunities to join our rapidly expanding community respiratory team as Band 6 Respiratory Nurses. The successful candidates will complement the existing Community Respiratory team which is multidisciplinary and includes Respiratory Nurse Specialists. We are seeking applicants with experience within respiratory / acute medical / community, at Band 5 or above and appropriate post registration qualification / relevant experience.

You will be expected to be an autonomous practitioners providing specialist assessment and treatment of patients with chronic respiratory disease who have complex needs and support them to self-manage their condition.

The service provides expert advice, guidance and information to patients, health and social care professionals, carers, relatives and other non-professionals with regards to respiratory care.

You will also support patients on the education component of pulmonary rehabilitation. As your experience grows and you develop the necessary competencies the role will progress within pulmonary rehabilitation delivery.

Main duties of the job

Manage a caseload of complex patients with Chronic Respiratory Disease including Chronic Obstructive Pulmonary Disease (COPD), Bronchiectasis and Interstitial Lung Disease (ILD) in the community.

To assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.

To provide a multidisciplinary approach to triage, support, treatment and when required to refer patients with Chronic Respiratory Disease onto other services as appropriate within the community setting.

Provide specialist advice, support and education to other health care professionals in the management and treatment of patients with Chronic Respiratory Disease

The team also has strong links with both Primary and Secondary Care liaising with Respiratory Consultants and Respiratory Specialists Nurses.

To participate in the delivering of organised respiratory educational sessions to health care professionals

On a rotational basis will support Pulmonary Rehabilitation (PR) delivering education sessions on respiratory exacerbation and Self-Management planning and other topics included in the PR programme which the post holder has competency to deliver. Additionally, with further training the post holder will be the qualified member of staff supporting the delivery of the exercise component of the PR programme.

The service works as part of a wider respiratory pathway for acute and chronic respiratory disease management and work is in line with the strategic objectives set out in the NHS Long Term Plan.

The Community Respiratory Service focuses on individual care planning and enabling patients to effectively manage their own care and therefore, reduce the number of emergency admissions.

Patient Care

  • To provide expert clinical management for patients with Chronic Respiratory Disease who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission
  • To provide clinical assessment, initiate medical or nursing treatment, refer appropriately and lead co-ordination of care, working with partner agencies and key professionals
  • Continuing responsibility for a caseload of patients of varying dependency needing complex Respiratory management.
  • Working across primary and secondary care and liaising with social care services as needed
  • Supporting and working with family members and liaising with carers
  • Undertake comprehensive assessment of the health and psycho-social care needs of patients with Chronic Respiratory Disease. Gathering and interpreting information and analysing the results.
