Job summary

We are looking to recruit a Care Co-ordinator to work within the Chesterfield Integrated Community Services and Rectory Road Surgery.

The purpose of the CST is to co-ordinate the care of patients within a GP Practice who have been identified as being at risk of their current situation breaking down leading to admission into hospital.

The role of the Care Co-ordinator is pivotal to the CST as it will be the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

The Care Co-ordinators are an administrative, non-clinical role, and we welcome applications from candidates with varied clinical/non-clinical experience.

We are looking for excellent communicators with a high level of organisational and IT skills who have the energy, enthusiasm and motivation to improve people's lives.

Main duties of the job

The Care Co-ordinator will be part of the 'Community Support Team' (CST) who are responsible for managing planned long term care. The Care Co-ordinator is a pivotal role to the CST and will be the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

To maintain IT-based information systems and take responsibility for the production of relevant performance data.

The Care Co-ordinator will contribute to tackling inequalities in health and social care particularly regarding individuals with long-term conditions. An ethos of promotion of independence and partnership-working is integral to this post.

About us

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
