Main Duties and Responsibilities
To provide professional and clinical leadership demonstrating a supportive and flexible leadership approach and a proactive responsible attitude so as to promote confidence in the team and Service.
To clinically supervise and line manage the UCR Team ensuring staff adhere to the Trust values and the NMC code
To work with and deputise for the Service Lead as required
To engage and lead on caseload management by overseeing, monitoring and evaluating care plans for all patients to ensure quality care, treatment progression and timely discharge.
To provide overview of remote monitoring systems within UCR and liaise with UCCH and VW Hub to support service delivery
To support the Clinician of the Day (COD) with delegation of tasks and the prioritization of work based on clinical acuity.
To work within the integrated team to prevent unnecessary admission to hospital and facilitate early discharge form hospital.
To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users
To support community clinicians and students through supervision, appraisal and mentorship in line with Trust policy.
To oversee that clinical supervision is occurring within the team and accurately recorded.
To liaise with the UCR Clinical Operations Manager (COM) to identify gaps in management and offer support as needed.
As part of operational leadership for the locality you will be required to:
Oversee the allocation of work for clinicians
Ensure all staff are competent with mobile working
Engage and lead on triage and referral to discharge standard processes
Work to a demand and capacity model which will reflect workload needs at any given time
Clinical
The post holder will oversee the following Core Clinical Interventions:
Assessment, planning, implementation and evaluation of all aspects of patient care, and development treatment/care plans that may need to be delivered from a range of options.
To develop new skills in care delivery, using innovative techniques and sharing these with the wider teams ensuring competencies are met.
Following a holistic assessment, devise an individualised care programme for each patient, modifying it as required
Assess patients holistic needs, communicating highly complex and sensitive information to patients and careers as to their assessment, diagnosis, prognosis and treatment plan
Become specialist in Assistive Technology and utilise it in patient care where appropriate
Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance
Be required to use tact and persuasive skills in order to gain the patients cooperation in their treatment/care/management plan.
Liaise with UCR, UCCH & Virtual Ward clinicians, GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately
To provide patient focused education, training and advice to patients, carers, other professionals and voluntary organisations.
To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework
Plan and organise specialist services within the team
Provide clinical expertise and advice to others, including Social Services, GPs and other agencies
Ensure activity data and clinical information is recorded on SystmOne and completed on a daily basis
May be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautions and adhere to infection control policies
Ensure informed consent is obtained prior to initiating interventions
Work with staff to ensure competencies are maintained
To maintain clinical credibility by acting regularly as a clinical practitioner in own area of expertise
Professional Leadership
The post holder will:
Be required to undertake risk assessments and act upon them appropriately
Be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs
Ensure own paperwork and records are maintained, and also those maintained by the team
Monitoring equipment and supplies via the electronic ordering systems used within the team
To work with and deputise for the Service Lead
Engage and lead on triage and referral to discharge standard processes
Actively contribute to integrated team meetings and other meetings where appropriate and chair meetings as necessary
Develop and maintain effective working relationships and communication with all members of the organisation, other professionals and agencies
Instigate, and actively participate with the Clinical Operational Managers in the development of the operational strategy and project management
Contribute to working groups within the Locality and organisation
Support all team members within the integrated team, respecting them, their roles and contributions
Ensure effective use of resources, both financial and human, by members of the team
Be aware of budget and ensure all effort is made to work within this
Participate in the recruitment process, using the opportunity to skill mix to meet the demands of the service
Attend training as required to develop current job role
Oversee safe use and maintenance of equipment and supplies
Remain accountable for own professional actions as determined by the professional bodies
Participate in evidence based practice, using research methodology
Produce induction programmes for students and new starters
Be accountable for work delegated to others within the team, ensuring staff are competent to undertake delegated work
Ensure working environment is conducive for learning and an appropriate learning experience for students
Provide mentorship for students undertaking pre/post registration course
Provide advice/training to team members
Mentor existing staff and provide training in specialist area as appropriate
Demonstrate leadership skills
To relay information from the Management team in respect of NCH&C and local issues to all members of the team on a regular basis in the form of business meetings.
To assist in the resolution of any conflicts, resolving informal complaints locally and assisting in the investigation of and response to formal complaints as required.
To participate in the staff appraisal scheme as both an appraisee and appraiser, maintaining a dynamic personal development plan.
Be aware of the Trusts behaviour framework and ensure behaviours are embedded in role and team and utilised when managing and supervising staff.
To actively contribute to the modernisation and forward movement of NCH&C, being actively involved in and taking an interest in changes within the NHC, the potential impact of local and national policies and the implications for the service.
To demonstrate a sound understanding of clinical governance and risk management, and apply to work situation, and ensure the team do likewise; identifying, assessing and documenting risk of implementing change
To actively perform absence management, submitting forms in a timely manner, as per the Trusts Policy
To be responsible for the compliance of Mandatory training and appraisals within the team, ensuring KPIs are achieved
To be an authorised signatory for goods, services and payroll.
To accurately complete personal human resources documentation in respect of team members. Ensure that designated time frames are adhered to.
Complete all data returns as required by NCH&C. Collate report, manage and analyse information on activity levels and quality, in accordance with agreed performance standards
To monitor the delegated team budget and assist the Locality Management Team to effectively manage resources
To monitor and manage staff absence, mandatory training, annual leave and study leave in line with NCH&Cs policies, procedures and guidelines. Inform the Locality Manager of any significant trends/issues.
To comply with existing organisational and departmental policies, procedures and guidelines and to be actively involved in the reviewing and updating of them as appropriate.
Ensure policies are interpreted correctly and that designated staff implement policy and service developments
To line manage UCR Band 6 and Band 5 nurses, as well as other clinical and non clinical staff.
