Clinical Nurse Manager Urgent Community Response Team

Job summary

We are looking for a dynamic, enthusiastic, resilient and dedicated Clinical Lead Nurse Manager to join our Integrated nursing and therapy Urgent Community Response team in North Norfolk, to support with the daily clinical and operational staff management. This is a fast-paced but rewarding role that would suit an experienced nurse who has a background in acute, community or significant experience in other urgent and emergency response teams.

This role will provide an opportunity for new challenges and on-going development in a multi-disciplinary innovative, supportive and friendly team who work closely with other community and NHS services to prevent admission to hospital.

The successful candidate will have excellent communication skills and work collaboratively to meet the needs of the service. It is essential that you have evidence of professional competencies, and are able to effectively apply your knowledge and skills to clinical practice and share your knowledge with the team in a range of ways.

Main duties of the job

Job PurposeTo provide clinical expertise, clinical leadership and line management to staff working within Urgent Community Response (UCR).

To support staff to achieve and ensure competencies are maintained within the services

To contribute in the delivery of high standards of health care to patients within their own places of residence

To ensure that Trust annual priorities and quality goals are embedded within the workforce by ensuring that all annual appraisals are complete within the team.

Hours of Operation

The service operates between 08:00 and 20:00 seven days a week and you will be required to work shifts between these times.

About us

You will positively contribute to service and quality development through new ways of working. The post holder will work alongside the Clinical Operations Manager and UCR nursing and therapy Leads to support with operational, clinical and professional development of staff and the service, who work across the North Norfolk area.

The UCR teams provide admission avoidance services via the Rapid Response MDP (formerly NEAT) and 2 hour UCR response process. We also support patients at the end of life and via the discharge to assess pathway.

Our service is participating in current and future innovative projects to support the on-going need to keep patients out of hospital and care for them at home when possible, and so you will be expected to contribute in supporting Virtual Ward patients and the recently formed award winning Unscheduled Care Coordination Hub.

To clinically supervise and line manage the UCR Team ensuring staff adhere to the Trust values and the NMC code

To work with and deputise for the Service Lead as required

To engage and lead on caseload management by overseeing, monitoring and evaluating care plans for all patients to ensure quality care, treatment progression and timely discharge.

To provide overview of remote monitoring systems within UCR and liaise with UCCH and VW Hub to support service delivery

To support the Clinician of the Day (COD) with delegation of tasks and the prioritization of work based on clinical acuity.

To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users

To support community clinicians and students through supervision, appraisal and mentorship in line with Trust policy.

To liaise with the UCR Clinical Operations Manager (COM) to identify gaps in management and offer support as needed.

As part of operational leadership for the locality you will be required to:

Oversee the allocation of work for clinicians

Clinical

The post holder will oversee the following Core Clinical Interventions:

Assessment, planning, implementation and evaluation of all aspects of patient care, and development treatment/care plans that may need to be delivered from a range of options.

To develop new skills in care delivery, using innovative techniques and sharing these with the wider teams ensuring competencies are met.

Following a holistic assessment, devise an individualised care programme for each patient, modifying it as required

Assess patients holistic needs, communicating highly complex and sensitive information to patients and careers as to their assessment, diagnosis, prognosis and treatment plan

Become specialist in Assistive Technology and utilise it in patient care where appropriate

Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance

Be required to use tact and persuasive skills in order to gain the patients cooperation in their treatment/care/management plan.

Liaise with UCR, UCCH & Virtual Ward clinicians, GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately

To provide patient focused education, training and advice to patients, carers, other professionals and voluntary organisations.

To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework

Plan and organise specialist services within the team

Provide clinical expertise and advice to others, including Social Services, GPs and other agencies

Ensure activity data and clinical information is recorded on SystmOne and completed on a daily basis

May be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautions and adhere to infection control policies
