Job summary
Be an active member of the integrated multi-disciplinary team for inpatients at Priscilla Bacon Lodge with palliative and end of life care needs.
Be responsible for a defined caseload, using evidence based/person-centred principles to assess, plan, implement and evaluate occupational therapy interventions.
To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
To support and deliver innovation within discharge practice and pathways.
To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from Priscilla Bacon Lodge.
Be responsible for the delivery and co-ordination of care to a named group of patients.
Main duties of the job
To work with the inpatients at Priscilla Bacon Lodge to assess their needs for equipment and discharge planning. To support what is important to each patient.
To support each patient who is being discharged with where they are to be discharged to, and support the patient and family with the transition of care from one clinical area to the desired destination.
to ensure all decisions are made with the patient and where the patient wishes with the family as well
To maintain own professional development and undertake opportunities for learning and developing transferrable skills to use in other clinical areas
