Job summary

The Norwich Community Therapy team are advertising for a friendly, flexible and conscientious Community Assistant Practitioner, band 4, to work in a supportive and nurturing environment.

Main duties of the job

The post holder will visit patients in their own homes and carry out a holistic assessment focusing on how they manage with mobility, transfers and other activities of daily living in their own environment. They will assess a variety of conditions and liaise with other health professionals working to enable the patient to be as independent for as long as possible.

A supportive induction programme will be offered. The post holder will receive monthly supervision with a registered Occupational Therapist or Physiotherapist. Training is encouraged.

  • oFoundation degree programme, or equivalent demonstrable knowledge and skills acquired through training and experience
  • oGCSE education, Grade C or equivalent in English and Maths
  • Experience of working in a health or social care setting

  • Experience of working with people at the end of life
  • Experience of working with people, in the statutory/voluntary/private sector.

  • oExperience of working in a community health care setting.
