Job summary

You will be a registered nurse with skills in palliative / end of life care, wound care, catheter care and urgent community 2hour response for patients and a strong desire to further develop your community nursing skills and experience.

You will be required to work over a 7 day working week including weekends and will be hard working, motivated, with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic assessment, to demonstrate that you use your initiative and have a keen interest in working with this group of patients. You must hold a full and valid driving licence and have access to a vehicle.

You will support staff working at a band 3 and 4 level, along with students and apprentices.

Due to the unplanned nature of the role we would recommend newly qualified staff to not apply.

Main duties of the job

As part of the Urgent Community Response (UCR) Team you will be required:o To work within the Urgent Community Response Team to prevent unnecessaryadmission to hospital.o To assess and prescribe care pathways for patients in a crisis situation withlong term conditions, the frail & elderly, palliative and end of life care and/orrehabilitation needs, to achieve quality of life and independence wherepossible, with support from Senior colleagues within the wider UrgentCommunity Response Team.o To act as the patients advocate, facilitating choice and patient empowerment.o To provide evidence based clinical/therapeutic interventions based on "bestpractice".o To work within the Urgent Community Response Team to support earlydischarge from hospital.o To work with all health care professionals, and statutory/non statutory agenciesto provide a seamless, integrated service to our service users.o To triage referrals on a rotational basis for the Urgent Community ResponseTeam.

About us

The post can be based at the new dedicated district nursing "Hive" building at the Norwich Community Hospital. You will be working closely with the other professionals with the urgent community response and virtual ward as well as link in with the planned community nursing teams. This dynamic and busy team require keen and motivated Nurses to provide assessment and quality interventions, in particular unplanned care to the population of patients restricted to receiving care in their homes.

ClinicalThe post holder will:1.Assess, plan, implement and evaluate all aspects of patient care, and developtreatment/care pathways that may need to be delivered from a range of options.2.Follow a holistic assessment, devise an individualised care pathway for eachpatient, modifying it as required.3.Be aware of Assistive Technology and utilise where required.4.Assess patients holistic needs, communicating complex and sensitiveinformation to patients and carers as to their assessment, diagnosis, prognosis andtreatment plan.5.Where there are barriers to understanding, such as hearing impairment, mentalcapacity impairment and other difficulties in comprehension, explanations requireadjustments in order to gain understanding, consent and concordance.6.Establish and maintain therapeutic relationships with patients and carers,incorporating motivation, encouragement and confidence building to enablepatients to engage in their treatment/care/management plan.7.Liaise with GPs, Social Services, inpatient teams, other allied health careprofessionals and the voluntary sector to ensure identified needs are met and careco-ordinated appropriately.8.Be required to undertake risk assessments and act upon them appropriately.9.Participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. GoldStandard Framework.10.Ensure activity data and clinical information is recorded on SystmOne andcompleted on a daily basis.11.Demonstrate dexterity and co-ordination when using specialist equipment andfine tools, advanced sensory skills, manual and mobilising skills.12.Order equipment and supplies via the electronic ordering systems used withinthe team.13.Ensure informed consent is obtained prior to initiating interventions.14.Be exposed to bodily fluids, infected material, blood products on a daily basis,therefore must utilise universal precautions and adhere to infection control policies.
