Job summary
We are looking to appoint a self-motivated individual with initiative, drive, strong interpersonal, admin and IT skills to work as a Palliative CareCoordinator based at St. James' Clinic, Exton's Road, King's Lynn
Coordination Duties: To work as part of the team for the Specialist Palliative Care Service liaising closely with clinicians to ensure a timelyresponse to referrals. Taking and recording referrals electronically and allocating care packages under the supervision of the lead clinician andworking alongside the integrated teams and a variety of organisations and care providers.
Call Handling Duties: To deliver an effective, safe and competent call handling service for callers to the Specialist Palliative Care Service whichincludes patients, their representatives and Healthcare professionals
Main duties of the job
The role is interesting and varied, where candidates require good communication skills with the ability to interact professionally with patients,relatives and other staff groups:
o Administrative support to clinical and non-clinical staffo Make and receive referrals from a number of agencies. Record referrals electronically on SystmOne and liaise with clinicians to ensurea timely response to referralso To be responsible for working with team leads and clinicians revising and prioritising packages of care, as requiredo Communicate effectively to care staff working within the service regarding patient needso Managing, monitoring and actioning tasks on SystmOne and emails received by the serviceo To carry out and prioritise a variety of administrative and clerical duties and undertake other general office dutieso Provide service users and professionals with information, assistance and signposting, as required
