Job summary

Pineheath Ward delivers quality, patient centred care within our 24 bedded unit, providing rehabilitation to a group of patients with varying complexities of needs including palliative care. We accept adult patients of all ages who have rehabilitation needs. Our patients come to us following medical, surgical or orthopaedic treatment, sometimes directly from their homes.

Opportunities exist for further training and development within the role. You will be joining an enthusiastic, supportive, friendly multidisciplinary team.

The post involves rotation between day/night duties, full/ part time posts are available. Our focus is on continually improving the quality of care we offer to local people and on improving access to that care, helping people to move seamlessly from one service to another.

Main duties of the job

1. To accurately and promptly carry out the instructions of the Registered Nurse (RN) who is professionally accountable for the delivery of care.2. To carry out duties as specified within the trust's Approved Core Task List for Clinical Support Worker (working within assessed competence)3. To report to registered staff any patient that gives cause for concern or any observed changes in a patient's clinical condition.4. To support patient's emotional well-being by supporting positive conversations, actively encouraging participation in ward-based activities5.To accurately document all care delivered to patients, including recording documentation electronically on trust systems maintaining confidentiality at all times.6. To respond appropriately to clinical emergencies to ensure the patient receives appropriate care. Be aware of role in emergency situations and what actions should be undertaken.7. To be aware of and adhere to all trust policies / procedures / nursing guidelines when undertaking any activities.8. To make patients welcome to the clinical area and participate in the admission process under the supervision of registered staff.9. To keep the clinical area clean and tidy maintaining a safe environment10. To adhere to trust infection control procedures.11. To support relatives under the guidance of a RN.12. To assist with the discharge process for patients directed by RN and or member of Multidisciplinary Team (MDT)

Job responsibilities

Communication1. To welcome patients, visitors, and relatives to the clinical area, providing assistance, support and information where required. Being aware of barriers to effective communication and respond appropriately.2. To answer the telephone, when necessary, in a polite courteous manner and respond appropriately within remit and pass on enquiries to the Registered Nurse.3. Communicate effectively within the nursing team, multi-disciplinary team and other departments to ensure that patient care is well planned and coordinated.4. To contribute to the handover of the care of patients within the team under the direct supervision of a RN maintaining confidentiality at all times.5. Report and record using the trusts systems (Datix) of all incidents, accidents, and complaints to the senior nurse on duty.6. Use the System One and ICE and other trust systems as required by the clinical area.

Management1. Take all reasonable steps to ensure the safe keeping of patients property in accordance with trust policy2. Promote a professional, welcoming and uncluttered environment, keeping all storage areas safe and clean and tidy.3. To share responsibility in the care and maintenance of all equipment in the clinical area.4. Ensure all waste and soiled linen is disposed of in line with trust policy.5. Participate in audit under the direction of the RN.6. Cleaning and checking of beds, lockers, oxygen, and suction equipment in the clinical area following the discharge of a patient and in preparation for admission.

Professional/Educational1. To ensure up to date with all mandatory (including if appropriate safeguarding and safeguarding supervision) training sessions.2. To have completed the Care Certificate or relevant experience in a care setting.3. To be flexible in relation to patient requirements.4. To continue personal development in relation to identified objectives.5. To supervise and support new staff and working with local induction programme6. Maintain up to date knowledge of changes in policies and procedures relevant to the post7. Participate in training and supervision sessions relevant to the clinical area this could include clinical competences where appropriate8. Ensure that attendance at an annual appraisal is maintained and document.
