Job summary
An exciting opportunity has arisen to join a new heart failure team in North Norfolk. The post will initially be as a 12 month fixed term contract or 12 month secondment within NCH&C. Working with the service manager for heart failure and a band 6 heart failure nurse to establish a limited service to heart failure patients in North Norfolk.
This is a pilot project to prove concept of the impact that community heart failure nurses can have on patient outcomes particularly admission avoidance and will be the basis of a business case for a permanent heart failure service for North Norfolk.
This post is a fantastic opportunity. This service provides expert advice and treatment to patients with heart failure . The aim is to improve the delivery of evidenced based care to this group of patients.
Main duties of the job
Coordinating the management of the service's referrals and case load.
Accepting referrals and managing a patient caseload according to agreed guidelines.
Delivering expert patient care and support in a variety of settings including nurse led clinics, home visits and telephone support.
Providing education and support to patients who are at risk of or have had, an acute cardiac event in order to reduce their risk of further cardiovascular events including clinical assessment.
Liaising with multi-disciplinary teams (MDT) including consultant cardiologists & GP's for the initiation & up-titration of beta blockers & other evidence based medications for patients on the caseload & to promote a seamless service across the primary/secondary care interface.
Providing an expert resource for Primary Health Care Teams to provide care and support for people with cardiovascular conditions including medication initiation & titration.
Assisting in the provision of training and education for patients, carers and professionals to develop the skills required to manage cardiovascular conditions effectively.

- Previous experience working within the NHS

- To have or be working towards first degree
- Independent Prescriber (or willing to undertake)
- Post-registration qualification relating to Cardiovascular Disease
