Job summary

We are looking to recruit a hybrid Clinical Operations Manager/Clinical Lead Nurse to provide professional and clinical leadership and line management for our South community nursing triage and allocation Hub. Our Hub is based in Wymondham and triages, coordinates and allocates work for our planned care community nursing teams based across South Norfolk, working alongside our PCN partners and colleagues.

You will work within a professional and dedicated team and have support from other Clinical Operations Managers and Clinical Leads working across South PCN.

You will manage the hybrid workforce and take responsibility for staff clinical competencies , professional development recruitment, monitoring, lone working and adherence to policies. You will also take responsibility for the data and safety quality monitoring of the team and will ensure that Trust annual priorities and quality goals are embedded within the workforce.

Main duties of the job

To provide clear line management, operational and clinical leadership to the community nursing teams and clinic based services within the locality portfolio.

To contribute to the clinical Operations co-ordination within the Operational HUB.

Engage with the function of the Operational HUB Engage with mobile working

Engage with SystmOne Optimisation

Engage with referral to discharge standard processes

To be aware of the demand and capacity model which will reflect workload needs at any given time.

The Operational HUB operates between 8am and 6.30pm and this role will require a flexible working pattern including some weekends, to fit service needs, including some Hub shift cover 8-6.
