Job summary
NCHC is looking for a highly motivated Respiratory Nurse to work within the Pulmonary Rehabilitation (PR) Team part-time. The multi-disciplinary team provides PR supporting exercise and providing education across Norfolk. Since Covid-19 the delivery of the service has been transformed. Face to face programmes are delivered in 2 locations (Kelling and Norwich) with an aim to expand to other locations in the future. Additionally PR programmes are delivered virtually via Teams, a homebased program via telephone, or via MyCOPD app. These new additions to delivering PR have been well received and evaluated by our patients over the year.
We are registered with the national Pulmonary Rehabilitation Services Accreditation Program (PRSAS) and are in the process of working towards accreditation which is seen as the gold standard for PR services. The service also participates in the National Asthma and COPD Audit Programme (NACAP).
Main duties of the job
Previous experience in assessment and treatment of respiratory conditions is essential and experience in Pulmonary Rehabilitation is desirable, however further training can be provided. There may be occasional requirement for some home-based working.
Some of the main duties are;
* To undertake comprehensive triage and assessment of people referred to the pulmonary rehabilitation service to determine their baseline and discharge clinical outcome measures within a variety of settings.
* To plan and deliver an individual specialist pulmonary treatment program using knowledge and evidence-based practice and treatment options.

We are a dedicated multi-disciplinary team, supporting the delivery of Pulmonary Rehabilitation (exercise, life style, lung education) for people who have a diagnosed long term lung condition in collaboration with the wider multi-disciplinary community pulmonary rehabilitation teams.
