Exciting opportunity for an experienced full- or part-time Occupational Therapist to join the growing Homeward integrated nursing and therapy team in a permanent post.
Homeward is a short-term admission avoidance step-up and step-down service, whose purpose is to make patients going through a health crisis safe to remain in their own home and avoid unnecessary admission to hospital and/or facilitate prompt discharge from hospital / in patient units.
Shifts are 8am-6pm (4 shifts per week), including weekend working on a rota.
Main duties of the job
We are looking for therapists who would enjoy the fast-pace and variety of an Unplanned community service. The role involves:
* clinical shifts:
- completing urgent, unplanned admission avoidance or discharge to assess first visits to assess and support patients in their own homes
- completing holistic assessments, treatment and prescription of manual handling, care, equipment and rehab needs, including risk assessments as required and short term follow up.
You will act as a point of expertise to the wider multidisciplinary team, providing supporting on complex cases, carrying out joint assessments, coaching and supervising junior nursing and therapy staff, supporting nursing and therapy students, and raising awareness of the use of Occupational Therapy in their work.
* Inreach support to patients in 4 procured health beds in a local Nursing Home
* working at the award-winning NEAT (Network of Escalation Avoidance Teams) at the Home First Hub.
Full induction and regular formal 1:1 support provided. Management support and support from established Homeward therapy colleagues available on every shift.
Homeward office is manned by Band 6 Coordinating nurse & Band 3 care coordinator 8am - 8pm 7 days a week, who provide safety checks and support.
