Job summary

An exciting opportunity has arisen for a highly motivated individual to join Norfolk Community Health and Care as a Band 4 Discharge Co-Ordinator at a new rehabilitation ward based at Bowthorpe Road Norwich . You will work with the multidisciplinary team liaising with family, patients and carers to plan and facilitate timely discharges from the unit.

The successful candidate will be part of a team of Discharge Co-Ordinator's, 1 based at each of the generalist inpatient units, with a peripatetic DISCO supporting all units. You will be managed by the Operations Manager and form an integral part of a small team. You will also work closely with the team and ward leaders at the new purpose built ward and will be based at Norwich Community Hospital, Bowthorpe Rd. The New Hospital will be opened in Spring 2024 - from the point of recruitment and during induction you will be based at another Norwich Community rehabilitation setting, which will be discussed at Interview, and will then move from here to the new unit when it is opened.

This post is a substantive for 30 hours per week over 4 days, Monday- Friday, no weekends or bank holidays.

Main duties of the job

Duties include: Making onward referrals to appropriate services. Liaising with patients and family. Forward thinking about discharging a patient from when they are admitted. Working within a multi-disciplinary team including Nurses, Doctors, Therapists, Social workers. Problem solving and attending daily ward meetings.

  • RSA III

  • Proven ability to demonstrate a systematic approach to prioritisation of work and cope under pressure to meet deadlines
  • Administrative experience related to service users or patients

  • Experience with day to day problem solving related to patients and service users
