Job summary
The Community Learning Disability Nurse (Band 6) role is suitable for a registered Nurse with post qualification experience of working with adults with learning disabilities. The post holder will need to be able to work autonomously, as well as part of the nursing and wider multi-disciplinary team. The role will involve providing face to face care for patients in the community, whilst promoting health facilitation and access to mainstream health services. Clinical supervision will be available and supported by a Clinical Lead LD Nurse.

Main duties of the job
To be responsible for a caseload of clients with complex needs, using evidence based/patient principles to assess, plan, implement and evaluate interventions.
To provide clinical supervision and clinical leadership to Band 3/4 nursing staff as required.
To be part of the planning, development and evaluation of LD Nursing service across the county to help ensure the consistency of high quality and efficient clinical services.
To support colleagues to provide practice placements for nursing students, as required.
To deliver services in line with the appropriate clinical pathways/specification requirements.
To act as a Care Coordinator for patients with complex healthcare needs who require ongoing monitoring and support due to the complexity and unpredictability of their health needs.
About us
The five Adult Community Learning Disability Teams in Norfolk are multidisciplinary teams, co-located with Adult Social Care. The post is with the Community Learning Disability Team in Norwich, based at County Hall. The team, which is fully integrated with our social care colleagues, is supportive, friendly and passionate about their work. We are currently working a hybrid model of office and home based, giving our employees more flexibility to get work done when/ where they're most productive.Our base in Norwich is very well situated access wise to the NDR, A47 and other major routes out into Norfolk. We are short walk into the beautiful village of Trowse, with Whitlingham Country Park offering lovely walks before and after work.
As a member of the team, you will have the opportunity to work collaboratively with a range of professionals and in a variety of settings that include: day services; residential services; supported living; and in people's own homes. Working in the learning disability service will enable you to gain experience in physical health; mental health; behaviour that may challenge others; complex health needs and comorbidities; continuing healthcare needs; psychological and emotional needs; and holistic nursing assessments of wellbeing in a person with a learning disability.
