Job summary
We are pleased to offer this permanent band 6 Physiotherapist post in Dereham (Mid-Norfolk) Community Nursing and Therapy Team based at Dereham Hospital, delivering services within the community and also the inpatient unit at Dereham Hospital.
This is a fantastic opportunity for an experienced Band 5 Physiotherapist looking to develop to Band 6 and those who would like to extend their leadership and managerial skills whilst guiding and supporting more junior members of the team.
We are looking for an enthusiastic, forward thinking, team player to join the Multidisciplinary team.
The Community Rehabilitation Service can offer and support opportunities for you to develop and progress along your chosen career path, whether that be clinically based, through the ECP/ACP pathways towards a band 8A ACP role, or a move into leadership and more senior management positions.
Your responsibilities will include managing a wide array of clinical conditions including Neurological, Orthopaedic (trauma), MSK community based conditions, Amputee Rehab, Respiratory and Elderly rehabilitation.
You will be working as part of an integrated team with Physiotherapists, Occupational Therapists, Community Assistant Practitioners and Healthcare Assistants, and alongside Social Care, Nurses, Community Matrons, Care Coordinators, ACPs and other specialist services.
The role is advertised as band 6 full time, but if suited for both applicant and service, part time can be considered.
Main duties of the job
This post provides an exciting opportunity for an applicant looking to build on a broad clinical skill base, who has excellent communication skills, along with complex problem solving skills to provide quality outcomes for patients. You will be expected to work in an integrated way with our partners in health and social care, whilst adhering to the Trust's Behaviour Framework. All team members are expected to carry their own clinical caseload, attend regular therapy meetings and undertake individual and joint therapy assessments and treatment sessions with members of the MDT where appropriate.
Rehabilitation is delivered to patients in community settings with a focus on improving health and well-being, working toward patient centred goals and preventing hospital admission or facilitating discharge home along the Discharge to Assess pathway.
You will be able to develop your knowledge and experience around Falls and Frailty and participate in the development of the Rehabilitation Service in the community. You will also have the opportunity to work with the unplanned Therapy team to prevent hospital admission and facilitate discharge.
You will be based in Dereham and have supervisory responsibilities for band 4 Assistant Practitioners within the Team, as well as students and rotational band 5 staff.
Car driver and own vehicle is essential to this post.
