Job summary

An exciting opportunity has arisen for a Band 6 Physiotherapist to be involved in the delivery of our Post Covid Service in Norfolk and Waveney.

This is a rewarding and challenging post in a new service which has been commissioned to assess, recognise, investigate, support and rehabilitate patients with Post Covid Syndrome.

Main duties of the job

Our post-Covid Service is working in line with national guidance, service specification and standards to offer assessments for people with new or ongoing symptoms 12 weeks after acute COVID-19, Investigations and referral, Planning care, Management, including self-management, supported self-management, follow-up and monitoring, service development, joint work with research teams and advice or signposting or offering rehabilitation.

We are looking for the right people for our team, who have a passion for patient centered, supportive care within a multi-professional team, with prior expertise, knowledge and training in complex, patient focused assessment, goal identification, coaching and self-management with a strong understanding of physical health and mental wellbeing and the possible symptoms and complications for patients in post-Covid syndrome.

To review referrals, support triage and liaison for patients referred into Post-COVID Service

Complete comprehensive and holistic assessment of patients referred to the service, undertaking a compassionate, non-judgemental, discussion with active listening skills

Supporting patients in acceptance of their changed circumstances encouraging patient focused outcomes, use of supported self-management and health coaching style advice on rehabilitation,

Supporting the MDT process and lead where required at Post-COVID Service, review patients holistic needs, communicating highly complex and sensitive information to referrers, and colleagues, patient and carers as required.
