Job summary

Key Areas of Responsibility

  • To work within the supported care team to prevent unnecessary admission to hospital.
  • To make risk assessments, establishing the safety or otherwise of a patient's ability to stay at home, or return safely from another care setting, providing and arranging as necessary supportive packages of care.
  • To provide evidenced based clinical/therapeutic interventions based on 'best practice' in order to improve health outcomes and promote choice.
  • To provide in-reach to inpatient units and step up beds where required
  • To act as the patients advocate, facilitating choice, patient empowerment and independence.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

About us

The Supported Care Service is a seven day service commissioned to provide an MDT approach to prevent unnecessary hospital admissions and to support timely discharges from acute and community hospitals. The team consists of Physiotherapists, Occupational Therapists, Nurses, Palliative Care Specialists, Assistant Practitioners and Health Care Assistants working closely with Norfolk First Support as part of the Network of Escalation Avoidance Teams (NEAT) and Discharge to Assess facilitation pathways.

This is an exciting opportunity for a Therapist that wishes to work in a rewarding, fast-paced service, where joint MDT working and decision making is essential in the effective management of this cohort of patients. There are plenty of learning opportunities within NEAT and the Supported Care Service to develop further skills in the management of the increasing complexity of patients within the community setting; so if you are interested in joining our team and developing your skills in a new and dynamic environment, then please consider applying.

Apply now to join an organisation that has been awarded an 'Outstanding' rating by the Care Quality Commission (CQC), the highest possible rating and the first stand-alone NHS community trust in the country to be awarded the title.
