Job summary

Are you enthusiastic and innovative? Do you work well in a multidisciplinary team setting? We are looking for a physiotherapist to deliver high quality interventions and health care to patients predominantly on Alder Ward, a community inpatient rehab ward based at Norwich Community Hospital. Key skills include actively engaging with patients and families, providing rehabilitation and planning for the next steps in the patients' journey. The role may also provide opportunities to work with patients in a variety of community settings, including providing outreach on discharge to follow up their care in their usual place of residence.

Main duties of the job

To assess and prescribe exercise programmes to adult patients with, but not exclusive to, frailty, complex comorbidities, palliative care and rehabilitation needs, to maximise quality of life and independence where possible.

To work with the integrated team to facilitate early discharge from hospital, and also to prevent unnecessary admission to hospital.

To work with health care professionals, statutory and non-statutory agencies to provide a seamless integrated service to our service users.

To provide advice, support and supervision to junior staff, therapy assistants and students.

You may also have the opportunity tosee patients in their own homes or in care homes. You will have regular clinical and management supervision. You will have senior management support on every shift. This role also includes a lot of opportunities for a wide variety of in house and external training.

MAIN RESPONSIBILITIES

Clinical

The post holder will:

Undertake comprehensive physiotherapy assessments of patients including those with a complex presentation, using investigative, analytical and clinical reasoning skills, and inform the development of a differential diagnosis.

Formulate and execute individualised physiotherapy management and treatment plans of patients including those with a complex presentation, in collaboration with the integrated team, other agencies and the patient, utilising a range of treatment skills.

Be aware of Assistive Technology and make referrals for this as required.

Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.

Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.

Be required to use tact and persuasive skills in order to gain the patients cooperation in their treatment plan.

Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.

To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.

Be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs.

Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.

Plan and organise specialist services within the integrated team, e.g. hip and knee group.

Demonstrate dexterity, co-ordination and palpatory senses for assessment, and manual treatment of patients, including using specialist equipment and fine tools, manual and mobilising skills.

Contribute to the requisitioning of supplies and equipment through electronic ordering systems.

Be aware of budget and ensure all effort is made to work within this.

Ensure informed consent is obtained prior to initiating interventions.

Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
