Main duties of the job

As part of the Fens & Brecks Primary Care Network team or Downham And Swaffham Primary Care Network team, you will provide nursing interventions for patients in their own homes and community settings. You will be able to work autonomously and in an integrated way with our partners in Health & Social Care. The ability to work as part of a team and to have an adaptable approach to work is essential.

Working within the team will include a variety of patients, conditions and tasks including:

  • Palliative Care - including syringe drivers
  • Wound Care - including Leg Ulcer pathways
  • Long Term Conditions - i.e. Diabetes management and insulin administration
  • Venepuncture
  • Catheter and Bowel care

We offer comprehensive training, supervision and you will be supported by a team of experienced Clinical Leads and Community Nurses.

The Trust provides a comprehensive preceptorship program for newly qualified staff.

Job responsibilities

  • To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
  • To work within the integrated team to facilitate early discharge from hospital
  • To work within the integrated team to prevent unnecessary admission to hospital
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

  • oDiploma or 1st level degree
  • oRN Qualification
  • oCurrent NMC registration
  • oENB 998/mentorship preparation or equivalent
