Job summary

We are looking for an enthusiastic and motivated individual with leadership experience, who can demonstrate creativity and innovation in their practice to ensure quality nursing support for the promotion of continence and management of incontinence for children aged up to 19 years in Norfolk.

This role will be based in the community setting (base to be confirmed in conjunction with the post holder) as part of a small integrated team and with a child/young person and family focus with opportunities to develop the service to meet local need.

Main duties of the job

  • The Specialist Paediatric continence nurse will usea person-centred approach, to develop, implement and evaluate continence management care plans in partnership with the child / young person, their families and carers whilst working in close co-operation with other clinical staff and disciplines.
  • To provide support, leadership and training to less experienced registered staff, non-registered staff and student nurses ensuring compliance to NMC standards to support learning and assessment in practice

  • Contribute to the safeguarding of children and young people.
  • To work with the integrated team to develop evidence- based practice by utilising research and auditing clinical effectiveness for the children with bladder and / or bowel dysfunction in the community
  • To identify and deliver on health education, develop and facilitate health promotion activities for individual children and their families/carers which minimise ill health and help attain optimum health potential

Job responsibilities

Clinical

1.1 Provide specialist support to clinical staff in the assessment and management of those children / young people aged 0-19 years who have more complex continence needs.

1.2 To demonstrate specialist knowledge from theoretical and practical experience.

1.3 To have specialist clinical skills to support the assessment of those children / young people with complex continence needs.

1.4 Be competent in reaching a diagnosis for patients continence symptoms and treat appropriately using clinical pathways and guidelines.

1.5 Refer children and young people to other specialists and agencies as appropriate.

1.6 To develop, implement and evaluate continence management care plans in partnership with the child / young person, their families and carers whilst working in close co-operation with other clinical staff and disciplines.

1.7 To assist in the education and training of registered staff, health care assistants and student nurses ensuring compliance to NMC standards to support learning and assessment in practice.

1.8 To support and train parents / carers in the correct use of identified medical equipment and management programmes. To foster confidence, competence and independence in families caring for their children at home.

1.9 Administer medications, treatments and interventions as prescribed ad to monitor their effects.

1.10 Work autonomously without direct supervision, accountable for own professional actions.

1.11 Keep timely, concise and accurate records, maintaining confidentiality and in line with Trust policy.

1.12 Provide direction, leadership and line management to junior members of the team.

1.13 Be able to communicate appropriately with children / young people, their parents or guardians.

1.14 Work alongside families and carers to provide care and support to meet the individual childs needs.

1.15 Develop and maintain good working relationships with staff supporting the needs of children with disabilities; school nurses, health visitors, occupational therapists, speech and language therapists, community childrens nurses, learning disability nurses, psychologists and all members of the integrated team.

1.16 Provide initial triage and assessment for new referrals to paediatric continence service.

1.17 Plan and allocate work in order of priority, delegating as necessary to the most appropriate Community Childrens clinical staff according to skill and competencies.

1.18 Contribute to the safeguarding of children and young people.

1.19 To provide a voice for children within an all age continence service ensuring the provision of quality care to children and young people in partnership with families and carers as commissioned and described within the service specification.

1.20 Ensure clinical care is evidence based and encompasses and integrated, holistic and team around the children approach.

1.21 Support the continued implementation of a Childrens integrated continence service in line with Department of Health and NICE guidance and National Service Framework for Children.

1.22 Support assessors of children and young people with bowel / bladder problems to be aware of and comply with related service guidelines and protocols.

1.23 Contribute to the continued planning, monitoring and development of this new service, ensuring that both service users and clinical staff are central to this regular audit process.

1.24 To have an up to date knowledge of paediatric continence products and equipment, allowing the trial and evaluation of new products on the market if suitable.

1.25 Contribute to the development of a clear process for the ordering and delivery of paediatric continence products for those paediatric clients 4-19 years, giving consideration to patient choice and client need alongside the requirement for an efficient use of resources.
