We have an exciting opportunity for an enthusiastic Senior Community Nurse to join West Norwich Community Nursing team.

The Community Nursing Service treats patients predominantly in their home settings, including residential homes and supported living complexes. We also provide some services in a clinic setting including our leg ulcer clinic and catheter clinic.

Norwich locality is working alongside its commissioners and local service providers as part of the Primary Care Network to develop community based services that support patients to remain at home and receive the services they require in an effective and co-ordinated way.

Main duties of the job

The workload requires a good range of clinical skills and nursing experience to be applied in managing our patients with varied health problems including diabetes management, traumatic and post-operative wound care, leg ulcers, pressure ulcer prevention and management, catheter support, palliative and end of life care. Many of our patients will have complex multi-pathologies and so we aim to provide holistic, patient centred assessments and deliver individualised care. As a senior community nurse you will support the Clinical Operations Manager in aspects such as roster creation, datix management, staff supervision, PDR's, and to develop a good working knowledge of the nursing caseload.

Norwich Place works as cohesive team of planned and unplanned nurses and clinics, as a senior community nurse you may be asked to support across teams and clinics according to the needs of the community nursing service.

To contribute in the delivery of high standards of health care to patients within their own homes, care homes and in the travelling community by providing proactive, holistic case management.

  • To assess and prescribe care packages for patients with long term conditions, the frail & elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
  • To work within the integrated team to facilitate early discharge from hospital
  • To work within the integrated team to prevent unnecessary admission to hospital
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
  • Contribute to the staff rota on a monthly basis.
  • Be required to undertake risk assessments and act upon them appropriately.
  • Contribute to the supervision of workload within the integrated team.
  • Be aware of budget and ensure all effort is made to work within this.
  • Actively contribute to multidisciplinary team meetings and other meetings where appropriate.
  • Support all team members within the integrated team, respecting them, their roles and contributions.
  • Provide advice/training to less experienced staff and supervise and support band 3 and 4 and 5 staff; and students where appropriate.
  • Mentor existing staff and provide training in specialist area as appropriate.
  • Contribute to the induction process for new staff/students.
