An exciting opportunity has arisen to join the Norwich Triage Hub Community Nursing Team, within the "Outstanding" Norfolk Community Health and Care. The positions is for highly motivated Triage Nursededicated to providing high standard clinical assessment and communication skills.
You should have proficient competencies and evidence-based knowledge in a wide array of nursing interventions to be able to triage effectively, support and coach patients and undertake clinical intervention when required.
The positionis split with 80 % of shifts undertaken in the triage hub to provide clinical support with patient referrals and triage. You will have a strong team work ethic, Excellent communication skills, sense of accountability and an ability to work under considerable pressure at times. The remaining 20 % of shifts will be clinical, based in the community to maintain competencies.

Main duties of the job
Enhanced Clinical assessment and prioritization of community patients, identifying their need for intervention and how soon this will need to be seen.
Enhanced communication and support for patients understanding their needs as well as being able to coach, support and signpost to appropriate services if this does not fall with in our service specification.
maintain adequate flow of work load from the triage lists to the community staff with in the neighborhood teams.
Main duties include.
To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
To document triage notes in order that the patient's record is complete and for audit purposes.
To use information technology to process the referrals for the 80% triaging role.
To liaise and communicate with the general public, other health care professionals,and multi-agencies on a daily basis.
To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achiever quality of life and independence where possible.
