Job summary

We are looking for an enthusiastic individual, RNLD preferably, however will consider RMHN with transferable skills, with an interest in working with adults with learning disabilities. The role will require you to work autonomously at times within the nursing team to provide assessment, treatment planning and intervention for patients.

You will have the opportunity to work collaboratively with a range of professionals and in a variety of settings that include: day services; residential services; supported living; and in people's own homes. You will gain experience in physical health; mental health; behaviour that may challenge others; complex health needs and comorbidities; continuing healthcare needs; psychological and emotional needs.

The role will include travel within a large rural locality, so a full and valid driving licence and access to a vehicle, or independent travel, will be essential.

Main duties of the job

  • To be responsible for the assessment of cared needs and the development implementation and evaluation / review of programmes of care with caseload responsibility and undertaking lead worker role as appropriate
  • To support health facilitation, health promotion and use of health action plans, whilst working in partnership with people who have a learning disability, carers, professionals and other agencies
