Job summary
Norfolk Community Health and Care NHS Trust is pleased to announce that an opportunity has arisen for an enthusiastic and motivated Physiotherapist to join our community in patient team at Kelling Hospital. Opportunity may also arise to help support our community teams seeing patients within their own homes. This can be a band 6 post or can be a linked grade providing you with the opportunity to progress from a Band 5 to a Band 6 in line with an agreed set of competencies.We are looking for someone who has completed 2-3 rotations or equivalent at Band 5 level.You will be supported in gaining skills in advanced clinical reasoning and problem solving as well as skills in complex assessment and intervention. Excellent interpersonal skills and the ability to discuss assessment and interventions empathetically with service users, relatives and carers is critical to this role, Support is available from Clinical Leads and Community Operations Managers. Band 6 competencies are expected to be reached within 12-18 months of appointment.

Main duties of the job
- You will be able to deliver high quality Physiotherapeutic interventions and health care to patients on community in-patient units. To provide proactive, holistic case management and rehabilitation whilst working with patients to help them set realistic goals. Utilising health coaching skills to effectively engage with the patient and their family to ensure that expectations are appropriately managed. This post may also have the need to out-reach into the community caseload as service needs' depends.
- To assess and prescribe care programmes for patients with, but not exclusive to, long term conditions, the frail and elderly, palliative care and rehabilitation needs, to achieve quality of life and independence where possible
- To assess and prescribe exercise programmes to patients with, but not exclusive to, frail ad elderly, palliative care and rehabilitation needs, to achieve quality and independence where possible.
- To work within the integrated team to facilitate early discharge from hospital
- To work with all health care professionals, and statutory and non-statutory agencies to provide a seamless integrated service to our service users
- To provide advise, support and supervision to junior staff, therapy assistants and students.
About us
You will work with a small team of friendly, approachable physiotherapists, occupational therapists, therapy assistant practitioners and ward registered nurses. The post will be based at Kelling Hospital Site providing cover on the 24 bed in-patient unit within Kelling Hospital. You may also be required to travel out to see patients in their own homes or in care homes in Fakenham, Wells, Holt and Sheringham and surrounding villages.
