Two fantastic Ward Manager opportunities have arisen in Norfolk Community Health and Care (NCHC) at Pineheath Ward, Kelling Hospital and Foxley Ward, Dereham Hospital.

Pineheath Ward and Foxley Ward are 2 of 6 community hospital wards within Norfolk providing frailty care, intermediate care, palliative care or generalist Palliative Support and Symptom Management. Each of the 6 community hospital wards are managed by a ward manager and 2 senior registered nurses with medical overview from Advanced Care Practitioners and Doctors.

Both Foxley and Pineheath have 24 beds with scope to increase to 28 beds according to system requirements. Both have established multidisciplinary team and strong connections with the local community.At NCHC we aspire to support and develop staff who have an ambition in the next 12 months to become a ward manager. This could include developmental Ward Manager posts (band 6 to band 7)with an existing Ward Manager assigned as a mentor and expectation to be working to the band 7 Ward Manager Job Description after a period of 12 months.

Main duties of the job

We are seeking an enthusiastic, dynamic, experienced and motivated nurse with a proven track record for organising and providing patient care of the highest standard.

The successful candidate will:

  • Be a forward thinking, innovative person with excellent verbal and written communication skills.
  • Have NMC and mentorship qualification
  • Be organised, highly motivated and engaged
  • Have considerable nursing experience in a band 6 role or ideally as a current ward manager.
  • Have experience of leading a multidisciplinary team
  • Provide excellent patient care
  • Have a excellent inter-personal skills to lead, work alongside and motivate the multidisciplinary team
  • Need to be motivated in implementing service changes and influencing others
  • Be expected to manage a team of staff members, deal with complaints at ward level and manage day to day operational issues

Norfolk Community Health and Care actively works towards improving patient care using national programmes eg Discharge to Assess and Accreditation. Across Norfolk and Waveney the National Hospital Discharge Service: Policy and Operating Model is being implemented which alongside the NHS Long Term Plan is placing greater emphasis on community services and supporting patients in their own homes. The community hospitals are part of the Community Beds and System Operations portfolio. Other services within this portfolio are the Operations Centre based at Norwich Community Hospital, the Inpatient Medical Team, and the Community Access Team based at NNUH who provide an admission avoidance service in the Emergency Department and lead on admissions into the community hospitals.   • To be responsible for the day to day operational line management of the team, leading and ensuring a 24 hour, 7 day service is provided.
  • To facilitate the discharge of patients to and from the community hospital and following Hospital Discharge Guidance including HomeFirst ethos.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to patients
  • To provide senior leadership Monday to Friday with the expectation to cover service gaps as required.
  • To ensure that patients, families and carers are involved in the planning and prioritisation of their rehabilitation care plans and discharge goal setting.
  • To provide supervision, advice and support to other members of nursing staff and rehabilitation assistants within the team regarding management and care of people with complex nursing needs
  • To lead on recruitment including inductions for new staff, work experience placements and other learners
  • To monitor and manage staff absence, annual leave and study leave in line with NCH&Cs policies, procedures and guidelines. Inform the Head of Service of any significant trends / issues.
  • Establish clear, positive and credible leadership within the clinical team, promoting and supporting the maintenance of a cohesive multidisciplinary team
