Job responsibilities

The post holder will deliver evidence based clinical care for patients with life-threatening illness who have complex specialist palliative care needs, from the point of diagnosis onwards under the supervision of the band 7 Community Specialist Palliative Care Nurses.The post holder will manage their own supervised clinical caseload, supporting patients and their families within their homes and out-patient/community hospital settings. They will linkclosely with the primary care team, the acute specialist palliative care team and the rest of the services within Priscilla Bacon Centre.

Person Specification

Qualifications

Essential

• NMC registered 1st level nurse
• Diploma in nursing and/or study to degree level or equivalent

Desirable

• Relevant post registration qualification in palliative care/ oncology
• Mentorship or equivalent teaching qualification

Experience

Essential

• Has had an understanding of specialist palliative care/oncology
• Ability to manage situations/environments

Desirable

• Knowledge to Health Care Policy and reform relevant to specialist palliative care etc Ambitions for end of life care
• Able to demonstrate evidence based knowledge of symptom control
• Previously worked in a palliative care setting inside or outside the NHS

Skills, Abilities and knowledge

Essential

• Computer literate
• Ability to communicate in the English language in both written and verbal formats
• Knowledge of advance care planning
• Awareness of current legislate / policy to end of life care
• Has ability to reflect on own practice
