We have an exciting opportunity for experienced and enthusiastic Band 5 nurses to join the Norwich Place Unplanned Care Community Nursing Team. Unplanned Care Team is a 7 day service, the service operates between the hours of 07:00 - 20:00

The Unplanned Care Team provides urgent, unplanned nursing support within 2/4/24 hours of referral to patients in their home settings covering Norwich, including in residential homes and supported living complexes, in order to avoid unnecessary admissions to hospital. The post involves lone working and managing own caseload throughout the day. Good communication with the coordinating team at Homeward and the ability to work as part of a multidisciplinary team is paramount.

We have an exciting opportunity for experienced and enthusiastic Band 5 nurse to join the Norwich Place Unplanned Care Community Nursing Team.

Main duties of the job

You will have a good range of clinical skills to be applied in assessments and management of our patients with varied health problems including diabetes management, traumatic and post-operative wound care, pressure ulcer prevention and management, male, female and suprapubic catheter insertion, bowel care, palliative and end of life care, syringe driver initiation, stat doses and verification of death. Many of our patients have complex multi-pathologies and so we aim to provide holistic, patient centre assessments and deliver individualized care.

You will have a proven background in nursing and demonstrate an understanding of the roles of planned and unplanned community nursing. Alongside the clinical responsibilities, the Band 5 Unplanned Nurse will support colleagues and other team members through supervision. NCHC is committed to the development of future health clinicians and there will be an expectation that the Nurse will support students in the role.

MAIN RESPONSIBILITIES

Clinical

The post holder will need to:

  1. Assess, plan, implement and evaluate all aspects of patient care, and develop care plans, using clinical reasoning skills, which may need to be delivered from a range of options.
  2. Following a holistic assessment, devise an individualised, evidence-based care programme for each patient, modifying it as required.
  3. Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
  4. Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
  5. Be required to use tact and persuasive skills in order to gain the patients co-operation in their care management plan.
  6. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
  7. Be aware of assistive technology and utilise where appropriate.
  8. To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.
  9. Ensure own caseload is planned and prioritised according to service targets and patient need.
  10. Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.
  11. Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
  12. Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
  13. Ensure informed consent is obtained prior to initiating interventions.
  14. Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
  15. Become competent in numerous nursing skills to provide urgent and unplanned response to patients to prevent avoidable admissions to hospital.

  1. Actively contribute to multidisciplinary team meetings and other meetings where appropriate.
  2. Support all team members within the integrated team, respecting them, their roles and contributions.
