Job summary

The Specialist Palliative Care service would like to recruit an experienced Specialist Palliative Care Nurse to support the existing team working in the central locality, based at Priscilla Bacon Centre

You will work primarily within the community but will work closely with the inpatient services, including the local Acute Trust.

As a band 7 you will be expected to role model and oversee band 6 staff members within the team which includes caseload reviews and regular peer support to ensure high quality patient care and support is being maintained.

Main duties of the job

You will manage your own clinical caseload whilst working corporately within the service, visiting patients within their homes and community hospital settings. The primary aim is to ensure continuity of care through all healthcare settings. In order to do this, they will link closely with the primary care team, Social Services and the services delivered within Priscilla Bacon Centre.

You will influence palliative care services at a strategic level as well as contributing to audit, education, and research within this area. It is acknowledged that this role requires a significant level of specialist knowledge and skills including clinical assessment skills and non-medical prescribing as well as mental and emotional effort.

You will act as an expert resource to all care professionals from health and social care services, independent and voluntary care agencies. This will include taking part in the 24/7 specialist palliative care advice line, which includes weekend working.
