We are looking for a School
Nurse with Specialist Community Public Health Nursing (SCPHN) qualifications, to
join either our public health or safeguarding team delivering the 0-19 Healthy
Child Programme.
Its an exciting time to join

health, offering consistency and support to our vulnerable families and young

Main duties of the job
You will need to have a broad understanding of children and
young people's health issues and be able to demonstrate flexibility and
creative thinking in respect of service delivery. Knowledge of recent documents
and strategies driving the development of universal and equitable services for
children is essential as well as understanding the principles of safeguarding
children and young people.
Excellent communication, interpersonal and leadership
skills are required with a commitment to team working as well as being able to
demonstrate working using own initiative. You will work closely with schools,
children and young people and their families, our partner agencies, and
commissioners to deliver a quality service.
You will be the named
School Health Nurse for an identified High School, college, and a cluster of
Primary schools. Manage the defined caseload, using a public health approach to
provide and maintain a high-quality service that promotes the physical and
mental health of the school aged population and their families.
Lead the delivery of the
Healthy Child Programme 519 and co-ordinate the delivery of public health
interventions for the post holders assigned schools under the direction of the
0-19 Team Leader. This will include health screening/surveillance health assessment,
immunisations, and a requirement to support the health needs of any vulnerable
children/young people within the caseload.

- The named School Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis.
- Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies
- Lead on the recommended health assessment and reviews including:
Reception/Year 1 (age 45) school entry assessment (transition from 05 HCP school entry questionnaire; Providing Year 6/7 (age 1012) assessment at transition from primary to secondary school.
- Supporting mid-teens reviews, when young people are embarking on the next transition stage as and when required in national and local specifications.
- Lead and coordinate a core programme of interventions according to agreed procedures, ensuring that Trust targets are met.
- Undertake comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care in line with the Healthy Child Programme 4-19 years of age.
- To contribute to the delivery of the national immunisation programme for school aged children.
- Plan, implement and evaluate interventions according to changing health needs to improve health outcomes. Consult, advise and educate children, young people and families to enable them to effectively manage their health needs.
- Perform home visits to children and families advising on all aspects of health as appropriate.
- Working in partnership with other key stakeholders to safeguard and protect children and young people.
- To contribute and take part in regular team meetings and contribute to multi-agency groups as appropriate.
- Working collaboratively to support children and young people where there are identified health needs, or where they are in the child protection system, providing therapeutic public health interventions for the child and family
- Provide training where necessary to young people, parents, teaching personnel and other professionals.
- Provide signposting to other services and agencies, referring and liaising appropriately.

- Organise, facilitate and participate in health promotional and health educational activities in schools and community settings, working in partnership with school staff and with other statutory and voluntary agencies.

- Complete PDRs and return to work pro-forma as required to the designated named staff within the team.
