Job summary

As a one to one for a child with a tracheostomy you will be working within the Childrens Community Nursing Team, delivering high quality evidence-based nursing care for a child with complex health needs within the nursery and home setting.

Main duties of the job

To work within the Childrens Community Nursing Service, providing high quality care for children and young people.

The post holder will provide 1 to 1 support for a child requiring tracheostomy care in the nursery and home setting. The post holder will support the childs parents/carers to provide care to enable their child to remain at home / access nursery.

The role will involve working as part a multidisciplinary team in particular working alongside staff within the educational setting.

The tracheostomy one to one will ensure that they work to the childs individual care plan, reporting any changes in the childs condition to the parents/carers and the Childrens Community Nursing Team.

  • Work to the childs individual care plan, reporting any changes in the childs condition to the parents/carers and the CCN Team.
  • To work in partnership with children and their families, undertaking care tasks as informed carers would perform.
  • To undertake aspects of care following training and the development of set competencies for children with complex, acute and long term health care needs.

Clinical Management and Care Delivery

  • Respond to the childs individual needs and support following care plans, implement and take part in the evaluation of care plans.
  • Be responsible for managing the childs airway via planned and emergency tracheostomy care in the nursery setting.
  • Provide care at the point of delivery in accordance with agreed competencies, policies and guidelines, recognising own limitations and seeking supervision and support as necessary.
  • Monitor the childs progress and improvement, against the agreed care plan.
  • Provide respite provision for children and young people with complex healthcare needs within the home or nursery setting.
  • Ensure the childs comfort, safety and dignity at all times.
  • Provide accurate care updates to childrens nurses regarding the childs physical and, emotional well-being and on their progress. Report any concerns or deterioration promptly to childrens nurses.
  • Assist the child or young person in the performance of activities of daily living.
  • Proactively safeguard the welfare of children and young people, actively adhering to safeguarding procedures and policies, maintain competencies by attending relevant training as required.
  • Maintain appropriate levels of equipment and supplies to ensure equipment is used effectively and economically.
  • Actively participate in the supervision and support systems within the team.
  • Provide accurate up to date activity data related to service provision, participating in data collection/audit as required.

Communication and Organisational Awareness

  • Promote good communication links between families, childrens community nursing and any other agencies involved in delivering care to the child/young person.
