is a service delivered in collaboration with our local authority partner.
Working in partnership with the bed based manager and therapy lead.
The nursing
team lead will:
Be
responsible for the clinical management of the Intermediate Care Nursing Team,
ensuring effective and efficient delivery of a high quality timely service to
meet commissioner requirements.
To undertake
highly complex clinical assessments and ensure the nursing team provides care
in line with Trust and professional body standards.
Plan capacity
and demand to deliver a safe service and contibute to admission avoidance and a
timely discharge home.
Motivate and
support the nursing team, inclusive
of supervision and staff development to
ensure a high standard of care and support to patients.
Carry out the
duties ensuring the service promotes equality and diversity, offering person centred care with dignity and
respect.
To be
responsible for the implementation of partner agency and Trust policies and
procedures.

To
