Job summary
We have an exciting opportunity for an enthusiastic and hardworking District Nurse Team Leader to join one of our friendly District Nursing Teams in Warrington. We are looking for highly motivated applicants who have a genuine interested in providing caring, effective and compassionate holistic care management, through assessments and evaluation of health needs in the community for patients in their home setting. You will have experience working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients promoting independence and wellbeing where possible. Having effective communication skills and being a team player is essential together with being competent in relevant nursing skills and knowledge.
Community nursing is evolving at a fast pace, and you must be able to engage positively with service improvements and change. There will be exciting opportunities to develop strong leadership and further clinical skills to ensure the delivery of effective evidence-based practice, clinical assessment, care planning and caseload management.
Main duties of the job
Main District Nurse Team Leader duties include:
· Perform holistic assessments of patient nursing needs. Plan, assess, implement and evaluate care delivery according to changing health needs utilising prescribing skills.
· Support the care management of complex/continuing care packages.
· Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.
· To establish and maintain effective communication with stake holders, individual groups and communities in complex and potentially challenging situations.
· Involve patients and carers in the planning and delivery of care.
· To competently perform a range of clinical skills related to the post.
· Maintain accurate records in accordance with NMC Standards.
· To participate in the health needs assessment of the practice population to identify areas of unmet need.
· Ability to work without direct supervision and to solve problems.
· Undertake chronic disease management reviews of the housebound patient using appropriate template
Work on a rota basis to facilitate weekend working across the district nursing teams
