We are looking for an experienced, values based, compassionate and driven professional to join Warrington Borough as our Intermediate Care Therapy Team Manager.

Warrington are well underway with creating an integrated Health and Social Care system through collaboration with partners to improve outcomes for communities within Warrington. Its an exciting time in our Intermediate Tier Services, we are on our transformation journey to develop our fully integrated and seamless services across our Intermediate Care. This is a pivotal role and offers an exciting opportunity to be key part of this transformation as we continue developing integrated Health and Social care in Warrington.

The Intermediate Care Therapy Team is part of the Integrated Care tier which includes the Intermediate Care Therapy Team, Urgent Community Response, Intermediate Care at Home, Care Call, Telecare and Sensory services. As the Team Manager you will be at the centre of our transformation work and will have the opportunity to shape and influence developments across the wider intermediate tier system

Main duties of the job

As the Team Manager, you will be responsible for day to day operational management of the team, including appraisals, supervision, absence management, annual leave, performance, service development, etc. You will ensure there is effective planning and co-ordination of the team, including caseload management, allocation of resources across various sites, ensure high standards of practice and compliance with policies and procedures, clinical governance, etc.

The Team Manager will provide visible and compassionate leadership, promoting a positive and innovative team culture, ensuring person centred high quality care. You will work closely with partners across the system, supporting the transformation and delivery of integrated health and social care services.

The successful candidate will be educated to degree standard or equivalent and have proven operational experience. You will be an excellent communicator, have great leadership and motivational skills and be happy to step outside of your comfort zone to drive forward changes

Duties and Responsibilities:

Leadership & Management

To develop a seamless service through close working relationships with all partners to ensure the needs of the service users are met in a holistic manner.

Work closely with key stakeholders to ensure that locality-based care pathways are developed and delivered as seamlessly as possible in an integrated manner.

Contribute to effective Case Management, Duty Management pathway planning and process review for the service and team members.

Ensure service users rights to make informed decisions, where necessary ensuring appropriate utilisation of risk management strategies and safeguarding procedures.

To ensure understanding, implementation and adherence of organisational policies and procedures by all staff.

Ensure appropriate incident reporting and subsequent action planning and management takes place.

Report to Operational Manager for all monitoring of budgetary matters relating to the service.

To monitor and report on monthly service data, taking appropriate action to allocate resources as appropriate.

To promote good team working and good working relations amongst staff.

To lead on recruitment, retention and staff development and training programmes to meet the needs of service and individual development plans.

To operationally manage and co-ordinate requests for annual leave, sick and study leave to ensure the service has cover.

To ensure and maintain high clinical standards throughout the service that comply with Care Quality Commission and relevant professional body standards.

To ensure all staff maintain appropriate and accurate patient records, in keeping with professional and departmental standards.

To maintain competency to practice through Continuing Professional Development.

To contribute to own CPD in a positive manner, evaluating one’s own practice and

To interpret policy documents from both government and professional bodies to contribute to planning, implementation and monitoring of outcomes.
