Halton 0-19 team are proud

develop the Family Hub model, working collaboratively with our partners and
families to co-produce services to meet the needs of our population.

to offer additional training in infant mental health, parenting support and
infant feeding in our collaborative approach with our partners.

Main duties of the job
Halton is a geographically
small area however provides a diverse community, ranging from affluent to socially
deprived. It is an area that provides excellent experiences for Health Visitors,
especially those that wish to specialise in specific areas.
The Public Health Child and Family Practitioner
will work within the 0-19 integrated public health nursing team to deliver a
flexible 8am-8pm service providing access to the Healthy Child Programme for children,
young people and families accessing the 0-19 service.
The post holder will
carry out individual assessments of health needs, implement and evaluate packages
of care as part of the Healthy Child Programme.

Duties and Responsibilities:
- To support the delivery of the Healthy Child Programme using evidence based practice to actively promote the health of children and young people.
- To conduct health needs assessments in a variety of settings and communicate the outcomes of assessment and future care planning to the named SCPHN.
- To promote positive mental and emotional health of children, young people and parents/carers.
- To initiate, contribute to and co-ordinate support packages to children, young people and families using the Early Help assessment framework as appropriate.
- To deliver and engage in health promotion programmes and public health initiatives in a variety of settings, including clinics, pre-school/schools and the community.
- Refer the child/families to other disciplines, professions, or agencies when appropriate to do so informing and discussing with the named Health Visitor/School Nurse/ PHCN.
- Support the organisation and delivery of 0-19 services core drop in sessions where the Public Health Child and Family Practitioner is deemed by the Team Leader/Health Visitor/School Nurse/ PHCN to be to be the most suitable person.
- Provide timely feedback to the named Health Visitor/School Nurse/ PHCN on work carried out in the Drop in clinics, School or home.
- Report to the appropriate Health Visitor/School Nurse /PHCN when there are any concerns about a child (this includes; physical/emotional or developmental condition of child, environmental and safeguarding concerns etc.)
- Adhere to Local Safeguarding policy and procedure to enable relevant referrals to be made to Childrens Social Care.
- Support delivery of all national screening programmes undertaken by the 0-19 service (e.g. NCMP)
- Provide Public Health Brief Interventions as identified for children and young people 0-19 years.
- Set up and run groups relevant to the communities needs and according to individual Public Health Child and Family Practitioners skills, (including carrying out risk assessments of premises etc.).
- Prepare health education displays.
Organisation of work
- Organise own workload effectively, ensuring a good use of time and an efficient and timely service to Children young people and families.
- Ensure electronic diary completed
- Submit accurate performance data on a monthly basis and in a timely manner.
Leadership
- Contribute to effective communications with patients, relatives and members of the 0-19 Team
- Attend and participate in team meetings.
- Attend professional meetings.
- Assist in induction of new staff within the skill-mix team.
- Engages with and contributes to the development of the service.
- Assist in research and audit projects as required.
