Job summary

It is an exciting time to join the Halton 0-19 Service. We are working closely with the Local Authority to develop the Family Hub model in Halton. Health Visiting is key to the delivery of the Healthy Child Programme. We are looking for an ambitious, dynamic SCHPN (Health Visitor) who enjoys working collaboratively to co-produce services for families of Halton. Within our 0-19 service here at Bridgewater we are dedicated to empower our teams to be innovative, encourage ideas to improve outcomes for our Children and Families in Halton.

An exciting opportunity has arisen to join our 0-19 Service which includes School Nursing, Health Visiting, Family Nurse Partnership and Immunisation Team. We are looking for enthusiastic and self- motivated individuals to work as effective and conscientious members of our Service. The role involves working as part of a skill mixed team delivering high quality care to children, young people and their families. You will work to deliver the Healthy Child Programme, within the Family Hub Model.

Main duties of the job

The successful candidate will offer the Health Child Programme, with mandated contacts antenatal visits, Birth Visits, 6-8 weeks, 1 year and 2 year assessment.

Safeguarding Children and Adults is a key part of the role.

The service consists of 2 teams, one based in Runcorn, and Widnes, its an exciting time as we are working closely with our Local Authority with the family hub model allowing for collaborative working with our partner agencies.

Halton is a small compact borough so travelling is minimised and our partners are within the close geographical boundaries.

  • To assess the health needs of children and their families, plan appropriate health improvement interventions with individual families and evaluate outcomes.
  • To offer direction, support and supervision to skill mix team colleagues in relation to delegated case work
  • To identify those children in need of protection, initiating Child Protection Referral when indicated
  • To participate in any proceedings required with regard to Trust Safeguarding Policy and procedure e.g. Case Conferences, Core Group and Child in Need.
  • To complete Court Reports as requested and adhere to time scales mandated by Court and attend/participate in any subsequent Court proceedings.
  • To provide information, advice and guidance on the management of health issues in a range of appropriate settings.
  • To identify minor childhood illness and as a Nurse Prescriber issue a prescription for treatment when indicated, appropriate and within the Health Visitors scope of practice.
